Fasting is often required for blood tests, medical procedures, or surgery, for example. All of these factors of intermittent fasting help with weight loss. The most common intermittent fasting diets consist of a hour daily fast, a hour fast on alternate days, or a two-day-a-week fast on nonconsecutive days.

Whatever plan you choose — after consulting your care team, including a dietitian who specializes in diabetes — should be tailored to your lifestyle, type 2 diabetes treatment regimen, and nutritional needs.

But attitudes have begun to change, and some newer studies suggest the approach may have benefits, including for people with diabetes.

For example, one study found that intermittent fasting lowered insulin resistance in 13 adults with type 2 diabetes. Insulin is a hormone that helps shuttle glucose sugar out of the bloodstream; people with type 2 diabetes are resistant to insulin, which results in higher blood sugar levels.

In addition, a review of research included several studies that found intermittent fasting reduced fasting glucose levels, weight, and post-meal blood sugar levels in people with type 2 diabetes.

Even with these studies, though, further research needs to be conducted to see whether intermittent fasting is truly safe for people with type 2 diabetes as a whole, experts say.

On the other hand, some experts say that intermittent fasting carries risks, especially for people who need to keep their blood sugar levels stable. For starters, skipping whole meals can result in poorer blood glucose control, not to mention issues such as fatigue and reduced energy — and, thus, an increased risk of injury.

Skipping meals may also cause people to make poor diet choices, which can have the opposite effect on their waistline and blood sugar. Intermittent fasting may also not be safe for elderly people with type 2 diabetes who have a reduced ability to endure hunger. When that happens, your liver responds by producing too many ketones, which can build up in the body and damage the kidneys as well as cause brain swelling, Pupo says.

Individuals trying IF should check their blood sugars more frequently — ideally, every 2—4 hours — especially at first. People with hypoglycemia should break their fast immediately and treat their low blood sugar with 15 grams of carbohydrates in the form of glucose tablets or gels.

They should consult a doctor before restarting the fast. It is also important to drink additional fluids during the fasting period to reduce the risk of dehydration and hypotension. A doctor may recommend stopping or reducing the dose of some diabetes medications, diuretics, and antihypertensives.

People should maintain a balanced diet on nonfasting days and avoid processed, fatty, and sugary foods. Doing so will mean that they do not reverse the positive effects of the fasting days.

There are a few diet takeaways from the science of IF. Insulin sensitivity changes with a circadian rhythm, decreasing throughout the day and into the night. Therefore, meals that a person consumes at night have an association with higher glucose and insulin levels.
