Omega-3s from fish and fish oil have been recommended by the American Heart Association AHA for the past 20 years to reduce cardiovascular events, like heart attack or stroke, in people who already have cardiovascular disease CVD. I have written about and been a strong advocate of getting omega-3s through diet, and sometimes through the use of supplements.

Over the past year I have prescribed an omega-3, sold under the brand name Vascepa, to my patients who are at high risk for CVD, based on evidence of cardiovascular benefits. Vascepa contains purified EPA, and its use is based on good clinical data from the REDUCE-IT trial.

This study enrolled over 8, patients with elevated cardiovascular risk and high blood triglyceride levels. They assigned half of the study participants to receive 2 grams of Vascepa twice a day, and assigned the other participants a placebo a pill filled with mineral oil.

The results showed a significant benefit of Vascepa over the placebo. Vascepa reduced blood triglyceride levels, but more importantly, it reduced the number of heart attacks and strokes, the need for a heart stenting procedure to open clogged arteries, and death.

In December , the FDA approved the use of Vascepa to reduce the risk of cardiovascular events in certain patients with, or at high risk for, CVD. But a recent study has raised some questions.

The STRENGTH trial , published in JAMA , looked at a different formulation of omega-3 fish oil — a combination of EPA and DHA — to see if it would also reduce cardiovascular risk. The trial was terminated early due to an interim analysis revealing no difference between the two treatment groups.

One possibility is that the different results are due to the different drugs studied. REDUCE-IT studied a purified formulation of high-dose EPA, which resulted in higher EPA levels.

This was similar to the results of another trial , which also found that pure EPA reduced the risk of cardiac events. The STRENGTH trial tested a combination of EPA and DHA. No large study has ever evaluated the effects on cardiovascular outcomes of purified DHA alone, leaving us to wonder whether DHA might counteract the benefits of EPA.

Back to my patients who want to know if they should be taking an omega-3 supplement. Eat a heart-healthy diet, get regular exercise, and pursue other lifestyle changes that have proven benefits for cardiovascular health.

What are omega-3 fatty acids and what do they do?

The ODS note that studies have found that people who eat fish, which is a key source of omega-3 fatty acids, typically have a lower risk of various long-term illnesses compared with those who do not eat fish.

However, it is not clear whether this is because of the omega-3s that the fish contain or something else. Furthermore, if it is because of the omega-3s that fish contain, it is not clear if a person taking omega-3 supplements will have the same benefits.

According to an article in the British Journal of Clinical Pharmacology , research has shown DHA and EPA to reduce the process of inflammation , which has links to various cardiovascular illnesses. However, while these effects were evident in animal studies, clinical trials on humans were less conclusive.

People with rheumatoid arthritis appeared to benefit from taking fish-oil supplements, but there was no clear benefit for people with inflammatory bowel disease or asthma.

According to the ODS , there is some evidence that taking omega-3 supplements may reduce the risk of a person having a heart attack. However, the ODS note that other studies did not find a link between omega-3 supplements and less chance of a person having cardiovascular issues in general.

A review article in the Journal of Physiology and Biochemistry states that this is a controversial area of research that is still up for debate. Triglycerides are fats, and if a person has an excess of these, they are more at risk of cardiovascular diseases. However, the NCCIH point out that medications that contain omega-3s among other ingredients have approval by the U.

Food and Drug Administration FDA to treat high triglyceride levels, although the same does not apply to omega-3 supplements. A study suggested that taking omega-3 supplements could benefit African Americans.

An article in The Journal of Nutritional Biochemistry states that research in humans is yet to show omega-3 supplements to help a person lose weight.

They may be able to help a person stop putting weight back on, however, although it is not clear precisely how they may do this. The NCCIH highlight a study that shows that the children of mothers who took a high-dose fish oil supplement were less likely to develop asthma than the children of mothers who took a placebo.

Some nuts, seeds and vegetable oils contain another omega-3 called alpha-linolenic acid ALA. Omega-3 fatty acids are essential for good health. Try to get them from your diet by eating fish — broiled or baked, not fried. Fish oil supplements might be helpful if you have high triglycerides or rheumatoid arthritis.

Fish oil appears to contain almost no mercury, which can be a cause for concern in certain types of fish. While generally safe, getting too much fish oil can increase your risk of bleeding and might affect your immune response. It's not clear whether fish oil is safe for people who are allergic to seafood.

Is grass-fed beef a good source of omega-3 fats? What is conjugated linoleic acid CLA? This is a type of omega-6 fat found naturally in dairy, beef, and vegetable oils.

It is also a popular dietary supplement, produced by chemically changing the structure of polyunsaturated vegetable oils. CLA supplements have been researched as a weight loss aid by reducing body fat; however findings have conflicted.

Some studies show a modest short-term weight loss while others show no weight changes. Some reported negative side effects include loose stools and fatty liver that may occur when taken in high dosages in supplements.
